Miami Herald editor Anders Gyllenhaal decided last week to use a photo of a doctor holding up a picture of a Vietnamese girl's 17-pound tumor -- "a picture in a picture, in other words, diluting the impact," writes Edward Schumacher-Matos. The next morning in an editorial meeting, some editors had second thoughts about whether they had been too cautious.
